MIDDLETOWN, NY – 10-13-2017 (Press Release Jet) — Nexus Health Resources, a leader in transitional care solutions, will be featured as part of a case study presented by Greater Hudson Valley Health System Chief Clinical Officer Rose Baczewski at the October 15-17 Marcus Evans National Healthcare CXO Summit.

Nexus Health is a key partner of Greater Hudson Valley member Orange Regional Medical Center in reducing unnecessary hospital readmissions. Nexus Health provides transitional care software, NexusConnexions®, in combination with its call center to Orange Regional for discharge phone calls. The case study will highlight the readmissions reduction initiatives that Orange Regional has implemented, including discharge phone calls, that have led to a greater than 60% reduction in overall hospital readmissions.

“We have a tremendous partnership with Orange Regional Medical Center,” said Nexus Health Resources CEO, Dr. Virginia Feldman. “Working as an extension of their discharge team our staff has assisted countless patients with ensuring they have the support and resources necessary to successfully recover at home. We’re pleased that the program pioneered by Rose Baczewski is getting this recognition and opportunity to share best practices.”

Nexus Health Resources is a provider of transitional care solutions for acute and post-acute providers. The company’s software, services and education assist providers with ensuring patients make a smooth transition to the home setting from either an acute or rehabilitation facility thus mitigating the risk of readmission.

“Reducing total medical spend through reduced readmissions has been a major initiative for Great Hudson Valley Health System,” said Baczewski. “Working with Dr. Feldman and our team we’ve been able to identify the most common points of failure that lead to readmissions and address them with a multi-prong approach of which the Nexus Health Call Center was a significant element.”

They are currently building a homestead from scratch on two little acres in central Texas. There’s a lot to be done to become somewhat self-sufficient, but they are debt-free and get to spend their days living this simple, good life together with their five young children.
